Know Your Hair Fall Before It Worsens

How Dutasteride Works: Two Enzymes Instead of One
Pattern thinning is a genetically influenced condition in which susceptible hair follicles progressively miniaturise under the influence of the hormone DHT (dihydrotestosterone). Testosterone converts into DHT through an enzyme called 5-alpha-reductase. This enzyme has three isoforms: types I, II, and III. Finasteride only inhibits type II, while dutasteride inhibits both type I and II isoenzymes. Since dutasteride works on both types, it reduces DHT serum levels by 90% as compared with about a 70% reduction with finasteride.
Finasteride switches off type II. Dutasteride switches off both, and it blocks type II 3 times more potently than finasteride does.
Then there is time. Finasteride has a half-life of around 5 to 8 hours, while dutasteride has a half-life of 5 weeks , so it builds up and leaves slowly. Dutasteride 0.5 mg is approved for pattern thinning in South Korea, Japan and Taiwan. In India, it is a Schedule H medicine cleared for the prostate and prescribed for hair fall at a doctor's judgement.

Formulations Available
Dutasteride is most commonly prescribed in oral form; however, topical form is also available. It can be administered locally through procedures such as mesotherapy.
Oral Dutasteride
The oral formulation was originally used for benign prostatic hyperplasia and is used off-label for treatment of androgenetic alopecia. A daily dose of 0.5 mg is used for the treatment of hair fall. It has superior effects in halting the progression of genetic thinning and stimulating hair growth compared with 1 mg finasteride. A 24-week study demonstrated that the mean change in total hair count after 24 weeks was higher in individuals receiving dutasteride treatment than in the finasteride group.
Topical Dutasteride
Recent dermatological insights highlight an innovative approach: topical administration with injectable dutasteride via mesotherapy or microneedling directly into the scalp. Injecting dutasteride directly into the scalp treats hair follicles locally, blocking DHT production while reducing the systemic effects often associated with oral pills, but evidence is limited.

Dutasteride vs Finasteride at a Glance

Who Dutasteride Actually Suits

What the First Year Actually Looks Like
The timeline for dutasteride is not similar to finasteride; most patients almost give up at Month 3 when the results are merely visible, and hair growth still looks the same. One major randomised trial assessed hair count and hair width at week 24 and found significantly greater improvements with dutasteride 0.5 mg than with finasteride 1 mg and placebo.
Real-world data highlights a longer timeline for visual transformation. A multicentre chart review across 5 Korean clinics found that significantly more men on dutasteride than on finasteride had improved a full pattern grade by 12 months, where the thinning was M-shaped recession or crown. For diffuse thinning across the top, the difference became more apparent by 18 months. Visible improvement may become apparent around 6 months, and a clearer gap by 12-18 months. It takes time, and individual results may vary.
Side Effects, and What Your Doctor Should Be Watching
The reported effects are similar to finasteride: lower sex drive, difficulty with erections, breast tenderness or enlargement, and low mood.
Real-world numbers agree. Across 535 Korean men on standard dosing, adverse events occurred in 7.6% on dutasteride and 10.5% on finasteride. These figures do not mean that dutasteride is free of adverse effects, but the observed rates were not higher than those reported with finasteride.

Any antibiotic, antidepressant, anti-anxiety or HIV medicine you take. Dutasteride interacts with several medications.
Your family plans, as the drug can cause abnormalities in the male foetus.
